Step One:

Please complete the Application and submit to regan.poynter@discovery.place to get your child's enrollment process started

Step Two:

Please complete the Enrollment Form.

Each line must be completely filled out per KDHE. 

Existing Students:
To complete the enrollment process, please follow Steps 2- 4

Step Three:

Please complete the KDHE Health Form.

Each line must be completely filled out per KDHE. 

Step Four:

Submit remining Enrollment Form to our enrollment director.
